Pre-Printed Address Label

If you are filing a paper income tax return, use the pre-printed address label we sent with your tax booklet if the information on the label is correct. If the information is correct, remove the label from the front cover of your tax booklet and place it over the name and address area of your tax return.

If you do not use a label, complete the name and address area on your form.

Do not use the label if any of the following applies:

  • Your name or address is wrong.
  • You and your spouse are going to file separate returns and both of your names or initials are on the label.
  • You are going to file a joint return and only one name is on the label.
  • You are filing a computer-generated return.

Do not make corrections to the label. Simply enter the correct information on the form.

If you use a tax preparer who is filing your return on paper, please take the label to the preparer and ask that it be placed on your tax form.

Using the label speeds up the processing of your return, including getting your refund to you sooner. It also reduces the possibility of error which could delay your refund. The label also assures that your local income tax is allocated to the correct local government.
