First-time Filers

If you are filing estimated taxes for the first time, you must complete and file Form 502D (Declaration of Estimated Tax) to establish your account.

When completing Form 502D, be sure to check the appropriate box at the top of the form so we can send you special vouchers preprinted with your account information. You can use these preprinted vouchers to make future payments
