Approved Substitute Maryland Tax Forms

To avoid delays in processing, it’s important that you use an officially approved state tax form.

Officially approved forms include a special number on the bottom left corner of the form, showing a two-digit number indicating the year the form was approved and a two-digit vendor identification number.

This information signifies our official approval and it helps us process the data accurately after the form is scanned electronically.

The chart below includes approved vendors, vendor identification information and approved scannable forms. The Recommended Vendors list includes vendors who use 2D barcode technology on their forms for faster and more accurate processing. The Other Vendors list includes those who do not currently use 2D technology.
